DIGITAL PLAN ADHESION CONTRACT

SMART HEALTH ATN PREVENTION





This contract regulates the adhesion to the ATN Prevention Individual Smart Health Digital Plan, which offers telemedicine services, emergency care and other wellness services, via digital signature and intermediation to the CONTRACTOR. Contractor already qualified in the contracting form.


1. SUBJECT OF THE CONTRACT


The purpose of this contract is to subscribe to the Individual Smart Health Digital Plan, ATN Prevention, valid for a capable, healthy and adult person, with the following services:


  • 24-hour Telemedicine Service for a period of 12 months (Albert Einstein Hospital or equivalent)
  • Two in-person emergency consultations per year at a private hospital in São Paulo, Capital
  • Costs of medications and therapies in emergency situations, up to R$400.00 per year
  • Reimbursement of up to R$500.00 for medications prescribed by a doctor, whether urgent or not
  • Access to partner gyms at no additional cost for a period of 12 months
  • 10 annual collective outdoor training sessions, with dates set in advance
  • One Basic Health purchase per month of the Atn Prevention Smart Food service for a period of 12 months*

2. VALUES AND PAYMENT METHODS


  • Annual cost: R$ 9,850.00 - Plan delivered for 12 months for this price. No other monthly cost
  • Payment in installments: Up to 21x on credit card or 3x on bank slip with 5% discount
  • Payment in full: 10% discount, totaling R$ 8,865.00


After sending the signature, the Contractor will receive within 1 hour, via email and WhatsApp, a payment link generated by the bank to complete the transaction on the credit card. This ensures greater security and protected data. The same applies to payment by boleto or PIX. Be aware of boleto and PIX for ATN PREVENTION's CNPJ: 55.957.133/0001-42



3. RULES FOR REIMBURSEMENT OF MEDICAL EXPENSES AND MEDICINES


  • Reimbursement of hospital expenses (in cases of urgency/emergency) and medications will be made upon presentation of an invoice and medical prescription sent to the email atnprevention.com.br
  • Reimbursement will be made within 10 calendar days after analysis of the documentation.
  • Payment will be made exclusively to a bank account in the name of the CONTRACTOR.
  • The refund request must be made by email: atendimento@atnprevention.com.br



4. PLAN WAITING PERIOD AND CANCELLATION


  • The plan's grace period is 10 calendar days after confirmation of annual payment, submission of the form and agreement to the online contract. In installments, it starts within the same period. The first installment must be paid on the same day via PIX and the others via bank slip. PIX, E-mail code: atendimento@atnprevention.com.br
  • Automatic cancellation will occur in case of default or non-compliance with the clauses agreed herein.


5. USE OF PERSONAL DATA AND INTERMEDIATION OF SERVICES


  • Some of the services are intermediated, provided by third parties and authorized by the CONTRACTOR based on this contract.
  • The CONTRACTOR expressly authorizes the intermediation of services on its behalf.
  • The CONTRACTOR also authorizes the use of his/her personal data for this purpose.
  • It also authorizes the outsourcing, sale or partial or full replacement of the services contracted here, without prior notice, maintaining the same characteristics as contracted and without any additional cost during the contractual period.


6. RESOLUTION OF DISPUTES, COMPLAINTS AND CANCELLATIONS


  • The CONTRACTOR may register complaints, request returns within the legal period of 7 days or send suggestions through the Solution 4.0 SAC and Compliance Menu on the website atnprevention.com.br
  • As a last resort, you can contact us via WhatsApp (11) 5108-3247, for informal communication purposes only, without administrative validity.
  • All official communications with administrative validity will be made exclusively via the email registered on the form and via the email atendimento@atnprevention.com.br


7. CONTRACTUAL VALIDITY


After reading, understanding and agreeing, the CONTRACTING party signs this contract digitally, following the steps below. This contract is subject to the rules of the Federal Constitution, Consumer Protection Code and Civil Code. The Contracting party declares to be capable, in good health and literate, in order to contract and use the services as agreed. This contract becomes valid from the date of signature and payment.
